The premium amount for the plan is determined by the difference between the plan’s costs of providing Part A and B and any supplemental benefits offered as well as the federal payment to the plan for Part A and B.
A percentage difference is determined by the plan bid and the maximum federal payment. That percentage difference must be used to offer extra benefits and to reduce the Part B premium.
The plan can choose not to cover costs of procedures or services not deemed medically necessary by Medicare regulations.
