Agent training is now available for the Medicare Advantage line of business. Please follow the instructions below to certify or re-certify as an agent for Baptist Health Plan in 2017.
If you are currently an appointed agent, please fill out and return the following:
- Attachment E
- Current E&O Copy
- Fraud, Waste and Abuse training (AHIP)
If you are not an appointed agent with Baptist Health Plan Advantage, please fill out and return the following:
- Attachment C
- Attachment D
- Attachment E
- Current E&O Copy
- Fraud, Waste and Abuse training (AHIP)

Please note, the agent training states that a face-to-face meeting is required to be certified. While Agents are welcome to attend the events, a face-to-face meeting to be certified is actually not required.
