- Anthem Link Pathway EPO plans have been discontinued

- All Anthem of KY Small group plans include the Well Being Solution Foundational Package, the Engagement Package 200, Diabetic Prevention Outreach Program and SmartShoppers
- Future Moms is changing to Building Healthy Families-Resources for Prenatal to Early Family Care
- Cancer Care Navigators: Navigators support members thru out their journey by offering 24/7 access to Clinicians, Sydney Community Digital Resource Tools, and Concierge Care App
- Behavioral Health Case Management: RISE Program-Outbound Engagement for Substance Abuse
- Back and Joint Pain Guide: Offers a 6-week Online Coaching Program to Manage Pain
- Diabetes Prevention Outreach Program: This is an online/digital program that focuses on cutting the risk of developing Type 2 diabetes through healthy eating and physical activity.
NEW FOR 2024-ANTHEM BALANCE FUNDING (LEVEL FUNDED PLAN):
- We will be offering Anthem Balance Funding Plans for 2024. These will be available to quote thru the small group quoting tool on the Producer Toolbox eff 11/01/2023 for a 01/01/2024 eff date
- Group must currently have a Level Funded Plan
- Group must have 10 or more enrolled
- $20,000 Specific Stop loss
- 110% Aggregate Stop loss
- 12/12 Coverage Period with a 90 day run-out
- 50% Surplus Refund
- $28 Commission
- Non-Erisa Groups are Eligible
- Anthem reserves the right to decline to quote
Reminder -Second Renewal Dates for KY ChamberAdvantage Plans:
- 07/01/2023 – 12/01/2023 renews 10/01/2024 (up to 15 months contract length)
- 01/01/2024 – 06/01/2024 renews 06/01/2025 (up to 17 months contract length)
