Baptist Health Plan enrollment deadline extended for off-exchange, individual business. The application deadline for individual business sold OFF the health insurance exchange has been extended to December 31, 2015.
For a January 1, 2016 effective date, applicants now have until December 31, 2015 to apply. Applications can be mailed to:

**Apps sent by mail must be postmarked no later than December 31, 2015.
Applications must include the initial premium payment. A check can be mailed with the application, or email finance@baptisthealthplan.com to set up payment with a credit card.
