Medical Benefit Change

1. 1. Complete the Renewal Change Form to identify any changes to your coverage elections. (included in your renewal)
      1. Change or add specialty coverages using the Specialty product selection part of the form.
   2. Complete the Employee Plan Selection From (included in your renewal) to identify any changes to the enrollment elections for your employees.
   3. Sign at the bottom of the Employee Plan Selection Form.
   4. Return completed forms to our quoting/sales team and we will get the changes submitted.
