ABC Association (Associated Builders & Contractors) is a medically underwritten association plan through Humana, and may help your employer groups bypass the ACA community rating. Kentucky groups of 2+ that are in the eligible SIC code list, and are members of the ABC association, qualify for this program. To request a quote, the following is needed:
- Agency must be a member of the ABC. Contact your AgentLink rep to discuss this option.
- Association Proposal Request Form
- Excel file that includes name, DOB, gender, coverage type, resident zip code and any dependents’ DOB that are to be included on the plan
- Completed ABC employee enrollment form
- Renewal letter
- Prior carrier bill
- Current plan design summary
Currently, there are 22 medical options and 5 dental options available through the ABC association; with the hope of adding a vision offering in the 3rd quarter. Please see below for relevant forms available for download:
